JOB PURPOSE
Under general direction, provides appropriate intervention (including counseling), complete psychosocial assessments, discharge planning and information and referral.
JOB RESPONSIBILITIES/DUTIES
· Collects information from clients and significant others to assess psychosocial, emotional and medical/financial needs relevant to the medical case management plan
· Provides information and referrals to service providers based on client needs assessment
· Accurately completes designated forms, agency reports and other written documentation within time limits defined upon assignment
· Performs other duties as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES
· Knowledge of treatment plans, conducting counseling in both single and group settings, and collaboration with other healthcare providers regarding treatment plans.
· Skill in clear and effective communication, writing and executing treatment plans, and seeking appropriate referrals as need for patients
· Ability to improve the health of patients utilizing all available resources
· While performing the duties of this job, the employee constantly is required to Grasping, handle, feel, lift up to 10 lbs., listen, see, sit, talk
· The employee frequently is required to Reach, stand, walk
· The employee occasionally is required to Pull, push, stoop
MINIMUM EDUCATION AND EXPERIENCE
Masters degree in Social Work (MSW) plus minimum two years experience in a field related to counseling individuals in crisis/trauma situations
REQUIRED CERTIFICATES/LICENSES/CERTIFICATES OF COMPLETION
Current Social Worker License (LMSW)
PREFERENCES
Knowledge in working with HIV population; Provide short and long term counseling to patients who are HIV positive; Interview patients and complete psychosocial assessments; Spanish speaking preferred
